Care Management by ElderCare Advocates

How long can Mom stay on her own? Where will she go when she can't manage any more? What can we afford? What resources are available? What will happen to her assets? And, of course: Will she have to move in with us?

Sooner or later, most of us have to face these difficult questions when one or both parents become frail and unable to cope with everyday tasks. When you find yourself in this position, you can turn to ElderCare Advocates.

ElderCare Advocates doesn’t provide the physical care, but we’ll guide you through the initial process and help you develop long-term solutions that you can afford – administering the process and the care where needed.

Here’s how we can help:

1) Presenting Options
As professional geriatric care managers, we know the range of choices available to frail seniors. We'll present the living and care options and discuss their relative merits.

2) Determining What You Can Afford
Based on the total income and assets available, we’ll make long-term cost projections for a range of care and living options for your parent. We'll help you pick a plan which meets the financial needs of the whole family.

3) Recommending Solutions
Once we’ve gone through the process, we can help you narrow the selections and make the best choices for you, your parent and your family.

4) Finding Resources
We'll guide you through the maze of governmental and social service resources available (including Medicaid) and find access to the services you qualify for.

5) Providing Objective Evaluation
We'll conduct an objective professional evaluation based on your needs, not on our preferences or relationships.

6) Managing Your Parent’s Care
We’ll develop a care plan, screen and hire the personnel, agency or facility to implement it and monitor the program on a regular basis. We’ll provide coordination with doctors, social services agencies, home health agencies or equipment providers. We can also help in filing medical insurance forms, keeping records and paying bills. And when the time comes, we can select a nursing home and monitor the care your parent receives there.

If the pressures of work or family prevent you from spending the time you would like managing your parent's care, let us help. Many of our clients are the remote families of area seniors. ElderCare Advocates is a perfect solution for families who live away from the area and can't visit regularly enough to provide adequate care management.
